Per la A penso sia più corretto usare present perfect simple con l'ausilio di for
"How long has it been since you last heard from Tom?
Per la B userei il present perfect continuous per evidenziare la durata dell'azione.
"I have not been hearing from him for three weeks
"How long has it been since you last heard from Tom?
Per la B userei il present perfect continuous per evidenziare la durata dell'azione.

Scusami, non bisogna scegliere tra since/you/last/hear ahahahah, bisogna formare la domanda utilizzando quei quattro sintagmi
How long is it you from Tom?
I haven't heard from him for three weeks
